Why can't I make an appointment after I click the book appointment tab?

This means that there is no vaccine availability. Please check back.

What should I bring on the day of the appointment?

Please print and fill out the vaccine form and bring to your appointment. Bring your ID and insurance card. Please bring in your CDC vaccine card so we may update it for you.
